Title: RN – Clinic/Ambulatory Job Description: Position Overview: As a Registered Nurse (RN) in a clinic or ambulatory care setting, you will be responsible for providing comprehensive nursing care to patients across the lifespan in an outpatient environment.
Your role will involve assessing patient needs, administering treatments and medications, educating patients about their health conditions, and collaborating with healthcare providers to optimize patient outcomes.
Key Responsibilities: Patient Assessment and Triage: Conduct thorough assessments of patients presenting to the clinic, including gathering medical history, assessing vital signs, and identifying current health concerns.
Perform triage of patients based on acuity and urgency, prioritizing care and facilitating timely interventions for those with urgent needs.
Nursing Interventions and Procedures: Administer medications, treatments, and vaccinations as prescribed by healthcare providers, ensuring accurate dosages, proper techniques, and adherence to infection control protocols.
Perform nursing procedures, such as wound care, dressing changes, suture removal, and specimen collection, following established guidelines and best practices.
Patient Education and Counseling: Provide patient education on health promotion, disease prevention, and management of chronic conditions, using clear and understandable language tailored to the patient's needs and preferences.
Counsel patients on lifestyle modifications, medication adherence, and self-care strategies to empower them to actively participate in their healthcare and improve their overall well-being.
Care Coordination and Collaboration: Collaborate with healthcare providers, including physicians, nurse practitioners, and physician assistants, to develop and implement individualized care plans for patients, incorporating evidence-based practices and patient preferences.
Coordinate referrals to specialty services, diagnostic testing, and community resources, ensuring continuity of care and comprehensive support for patients' healthcare needs.
Documentation and Communication: Document all assessments, interventions, and patient interactions accurately and timely in the electronic health record (EHR) system, maintaining confidentiality and privacy.
Communicate effectively with interdisciplinary team members, including providers, medical assistants, and administrative staff, to ensure coordinated and efficient patient care.
Quality Improvement and Compliance: Participate in quality improvement initiatives, performance improvement projects, and regulatory compliance efforts to enhance patient outcomes and satisfaction.
Adhere to clinic policies, procedures, and regulatory standards, including infection control protocols, patient safety initiatives, and professional nursing practice guidelines.
Qualifications: Valid Registered Nurse (RN) license issued by the state board of nursing.
Bachelor of Science in Nursing (BSN) degree preferred.
Certification in Basic Life Support (BLS) required.
Minimum of 1-2 years of nursing experience in a clinic or ambulatory care setting preferred.
Strong clinical assessment and critical thinking skills, with the ability to prioritize and manage patient care in a fast-paced environment.
Excellent communication and interpersonal abilities, with a patient-centered approach to care.
Proficiency in computer skills and electronic health record (EHR) systems used for documentation and patient care management.
