Responsibilities
Works in conjunction with the Practice Manager to coordinate and oversee the activities of the clinical team members engaged in patient care for the diagnosis, treatment and prevention of disease.
Provides case management by being a patient advocate, resource and contact person for patients to improve clinical quality and clinical documentation
Evaluates the effectiveness of the care provided by interdisciplinary care team; identifies problems and guides team members to their solution.
Identifies barriers in care and outcomes or clinical concerns and communicates with patients, staff and providers to formulate plan of care and ensure compliance and continuity of care
Qualifications
Minimum:
Two years RN experience in a medical setting taking vitals, assisting with medical examinations, completing patient assessments and care plans and working on quality measures
Must hold current valid Arizona or compact RN state license.
American Heart Association Basic Life Support for healthcare provider certificate.
Basic computer literacy and proficiency
Preferred:
Electronic Medical Record (EMR) and/or Cerner experience.
Pay Range
$33.
