Overview
We are seeking a highly motivated and experienced Healthcare Navigator to join our team at Polara Health in collaboration with the Federally Qualified Health Center (FQHC).
The ideal candidate will be a Licensed Practical Nurse (LPN), Medical Assistant (MA), or Behavioral Health Technician (BHT) with a strong background in both healthcare and behavioral health.
This position generally works M-F, 8 am to 5pm.
Essential Duties
Patient Coordination
Collaborate with healthcare providers at Polara Health and the FQHC to coordinate physical and behavioral healthcare services for patients.
Ensure smooth communication and collaboration between different healthcare teams to provide integrated care.
Navigation and Enrollment
Assist patients in navigating the healthcare system, explaining available services, and guiding them through the enrollment process.
Conduct assessments to determine patient needs and connect them with appropriate services.
Engagement and Education
Engage with patients to understand their healthcare preferences, concerns, and goals.
Provide education on behavioral health and physical health services, empowering patients to actively participate in their healthcare.
Advocacy
Advocate for patients to ensure they receive timely and comprehensive healthcare services.
Address barriers to access by working closely with healthcare providers and community resources.
Appointment Scheduling
Assist patients in scheduling appointments for both behavioral health and physical health services.
Follow up to confirm appointment attendance and address any scheduling conflicts.
Resource Referral
Provide information and referrals to community resources that support patients' overall well-being.
Collaborate with external agencies to connect patients with additional support services.
Documentation and Reporting
Maintain detailed and accurate records of patient interactions, assessments, and service coordination activities.
Generate reports on patient engagement, outcomes, and service utilization.
Crisis Intervention
Recognize and respond to potential crises, providing immediate support or facilitating access to emergency services.
Collaborate with behavioral health professionals in crisis situations.
Follow-Up and Follow-Through
Conduct regular follow-up with patients to assess progress and address any ongoing healthcare needs.
Ensure continuity of care by facilitating communication between healthcare providers and patients.
Quality Improvement
Participate in quality improvement initiatives to enhance the effectiveness of healthcare coordination and patient outcomes.
Provide feedback to the healthcare team on areas for improvement in service delivery.
Licensed Practical Nurse (LPN), Medical Assistant (MA), or Behavioral Health Technician (BHT) preferred.
3 years clinical experience, and/or bachelor’s degree in related field.
Strong understanding of the healthcare system and services available for behavioral and physical health.
Excellent communication and interpersonal skills.
Ability to work collaboratively in a team and independently when needed.
Empathy and sensitivity to the diverse needs of patients.
