POSITION SUMMARY The Licensed Practical Nurse/Vocational Nurse (LPN) is responsible for providing skilled nursing care under the direct supervision of the Registered Nurse (RN).
They perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care.
They are responsible and accountable for making decisions that are based on the individual’s nursing experience and educational preparation.
QUALIFICATIONS Current Arizona State LPN License Graduate of an accredited practical nurse or vocational nursing program.
Has one (1) year of nursing experience.
Community/home health or medical/surgical experience is preferred.
Possess and maintain valid BLS certification.
Complies with accepted professional standards and practice.
Demonstrates good verbal and written communication, and organization skills.
Licensed driver with an automobile that is in good working order and insured per the organization's requirements.

Our goal is to provide quality care in your home, enhancing independence while minimizing the effects of illness and disability.
Our services are prescribed by a physician to help medically qualified individuals with rehabilitation after an injury, hospital stay, surgery, or an illness.
Our services may include wound care, IV therapy and PICC line maintenance, cardiac or orthopedic rehab, medication management, pain and symptom control, or help managing a chronic medical condition.
Care is provided through intermittent home visits by specialized providers and continues until goals for recovery have been met.
