NEW HIGHER PAY RATES! We are currently seeking an experienced: HOME HEALTH NURSE – LPN Full Time or Per Diem Job Description: Provides direct patient care, to include using a variety of skills and necessary coordination of services.
Completes appropriate documentation as required by policy and procedure and regulation.
Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
Coordinates homecare services with the Registered Nurse, and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
Accepts and transcribes physician’s orders as directed.
Forwards all clinical, progress, and summary notes to the Registered Nurse for coordination.
Attends and participates in case conferences.
Job Requirements: Successful completion of nursing education from an approved school of nursing.
Current LPN licensure in Arizona and one year of clinical nursing experience, preferably in home health/hospice.
Ability to independently organize work and provide effective health care services to homebound patients.
Ability to ensure appropriate patient care as supervised by the home health nurse – Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
Effective communication skills (oral and written).
CPR certification.
