The Healing Place Health Unit Coordinator serves as a main facilitator, coordinator, and communicator to ensure smooth operations for the hospital program overseeing and performing non licensed clerical functions that best support the provision of services, effective processes and documentation during transitions in care, and ancillary support for the direct care and licensed team members to ensure orderly daily operations through assigned tasks.
The Health Unit Coordinator position reports under the direction and supervision of the THP site manager in collaboration with hospital nursing leadership.
Excellent communication and organizational skills, with the ability to multitask, and being self-directed are key attributes for role function.
ESSENTIAL RESPONSIBILITIES Receives and coordinates referrals between community facilities and Polara facilities ( THP-CSU-Connections Center ) for new and direct admit/fast track admissions , ensuring a timely and effective process to include facilitation of nurse to nurse report, required components of patient registration, establishment of the new patient chart and forms, and establishment of the patient in required systems to include the new admission account in the EMR.
Answers telephones, transfers calls to appropriate department staff, take messages, and responds to inquiries for routine information Assists physicians and providers as requested to support rounding activities and access to needed forms, charts, etc.
Maintains clean and orderly environment within assigned unit and supply rooms Ensures that hospital staff receive necessary notifications and messages Maintains, orders, and stocks to par inventory of patient care supplies, ensuring no expired supplies: toiletries, laundry detergent, scrubs, linens, lab supplies, snacks, office supplies Updates paper forms as directed and maintains par supply to meet unit needs Provides notary services for providers, social workers, and nurses for any legal documents, including COE/COT paperwork Sets up new admission patient chart, prints patient labels, and new admit forms packet for nursing admission, creates rounding form and any forms utilized for a new patient, and ensures timely and accurate registration in the EMR to include patient face sheet demo graphic information, insurance or payor source as indicated, patient photo, and other data required to establish a new patient record Checks patient records for completeness and accuracy, identify and facilitate process of making necessary changes with specific disciplines Assists in the facilitation of the patient discharge process to include creation of discharge packet for nursing discharge, updates in the EMR medical record, assists social work and nursing as directed for confirmation of follow-up appointments, post-discharge prescription medication availability at identified pharmacies, and other discharge tasks and documentation as needed Disassembles medical records following discharge and scans appropriate hardcopy documents to electronic health record and Health Information Management Department Completes coordination of care for patient discharges by faxing discharge summaries and 4.

1 forms to outpatient care providers Notify OP of all enrolled inpatient discharges Opens, sorts and distributes incoming mail Submits work orders to plant operations and IT and coordinates the follow up shift to shift to ensure completion for unit equipment repair needs as indicated.
Assists with arrangements for patient transportation as needed Assists mental health tech team with the check in process for patient visitors during scheduled hours as indicated Monitor lab process: Follow up on lab draws, print lab results, deliver to provider for review and signature, and file in patient chart.
This include the A1C and lipid results.
Completes and submits monthly critical lab logs to Nursing Leadership Submits monthly refrigerator temp/cleaning logs to QM Completes other duties as assigned Assists in orientation, and in-service training as delegated by leadership Other Key Competencies: Exercises a high degree of ethics and maintains strict confidentiality in handling highly confidential information with professionalism Must be able to work independently as well as collaboratively within a team setting Must have strong interpersonal communication, problem solving, and teambuilding skills Must be detail oriented, have excellent organizational skills and be flexible Must demonstrate clinical competencies essential to effective job performance Minimum of 1-year prior hospital experience or in like health care/ medical field role 1 or more years in acute behavioral health hospital setting preferred High school diploma required Previous electronic medical record experience preferred Must possess the ability to learn and/or operate relevant computer operating system environments and the ability to operate a variety of office equipment (fax machine, copier, scanner, computer, telephone system, postage meter, etc.
